Event Description:
There is a growing recognition of the need to support healthcare
practitioners in undertaking CVD risk assessment and effectively
managing and supporting high risk individuals.
Following the publication of “Putting Prevention First”
(Department of Health, 2008) the current move towards using an
evidence based approach to prevent or delay CVD offers an
exciting opportunity for healthcare practitioners who are trained
and have the knowledge and skills to deliver comprehensive CVD
risk assessment to their patients.
This workshop will give you the knowledge and skills to:
• Use current evidence based guidelines
• Understand modifiable and non modifiable risk factors
• Use risk assessment tools in practice to calculate CVD risk
• Be familiar with the investigations required for measuring
CVD risk
• Use motivational strategies to support behaviour change
• Understand the organisational structures required for
delivery of the Vascular Prevention Programme in practice
This is a one day workshop with informal lectures and interactive
practical sessions delivered by expert practicing clinicians

We are planning a joint educational meeting across various aspects of cardio-vascular disease entitled “From Cradle to Grave” at Ramside Hall Hotel on Thursday 18th March 2010.
This is a joint meeting organised by the North of England Cardiovascular Network (NECVN) and the National Primary Care Cardiovascular Society (PCCS) as well as the GPs with a Specialist Interest in Cardiology National Forum.
We have a collection of local and national speakers including Huon Gray who is the Deputy National Director for Heart Disease. This event is open to primary care teams including GPs, practice nurses and specialist nurses as well as secondary care teams and specialist nurses in the community.
We attach a draft programme for your attention and would be most grateful if you would be kind enough to make an effort to come along to this meeting. The numbers attending will be limited to 250 so please register as soon as possible to avoid disappointment.

Aim of course:
To develop the awareness and understanding of this often mis-understood issue.
To recognise that self-harm is usually a coping strategy which people may have developed as a way of expressing or relieving intense and overwhelming feelings and emotions.
To help carers and professionals working in a wide range of services, including health and mental health, education, youth work, drug & alcohol services, support services and counselling services, feel more confident in supporting those people who self-harm and where to get /refer to more specialised support.
Trainer:
Jill Eastham brings to the training her own personal experience of using self-harm to cope with the trauma of reliving painful memories of childhood sexual abuse. She offers part of her own ‘story’ to help you understand the individual meaning and purpose of her own past self-harm and describes the factors that were helpful to her in beginning to reduce and eventually stop that need to self-harm. As a professional counsellor, Jill developed Self-Injury Support in North Cumbria (SIS) into a unique independent service that was valued for its expertise and person-centred ethos. Jill offers professional understanding and insights from her work with the charity to further your knowledge around self-harm. In particular NICE guidelines, case studies and examples of care management are included.

On these evenings you will also have the opportunity to participate in a Research project looking into the Sessional GPs (locums and salaried GPs) and the professional support they require and receive. This project is particularly interested in experiences of Self directed learning groups.
There will therefore be a 1hour Focus group between 7and 8pm on both evenings (Food from 630pm) on the subjected of “support for sessional Gps and Self directed learning groups).

Self directed learning groups (SDLG) are defined as groups of GPs who meet regularly for the purpose of mutual support and continuing professional development, without regular external paid or unpaid facilitation]. We would be very interested in learning what has in your view been successful and worked well and what areas have worked less well. We would also be grateful if you would PASS THIS EMAIL ON TO ANY OF YOUR COLLEAGUES WHO ARE CURRENTLY, OR HAVE PREVIOUSLY BEEN, INVOLVED IN THESE KINDS OF GROUPS. This is the first time any formal research has been carried out to look at these organisations, the support they provide to sessional GPs, and models of practice. All information will be confidential to the Research Team and anonymised for the purpose of the report.

The most effective reversible contraception is the Implant. It is well known
that using more LARCs can be an important driver to reduce the abortion
rate in the UK. All practices should be providing information about this
method and most practices should be providing an insertion and removal
service on site. This session will cover the theory and model arm training
for Implanon. After this session, practical training for interested
practitioners can be arranged at the 1 to 1 centre.

2) PHYSICAL ILLNESS IN PATIENTS
WITH MENTAL HEALTH PROBLEMS -

Physical health problems are more common in people with mental illness,
and especially cardiovascular and metabolic diseases. A large study of
physical co-morbidity has recently taken place in North Tyneside
Community Mental Health Teams. The findings of this study will be
discussed, and strategies for improving monitoring and intervention will be
explored.

3) SAFEGUARDING CHILDREN

serious case reviews. The cases will cover child safeguarding definitions,

harm or exploitation. We will look at how vulnerable adults can suffer
abuse, how to recognise it is happening and how to respond and refer.

3 Management of Chronic Constipation in Adults

Chronic constipation in adults is a very common condition seen in primary care. Management can be difficult especially with co-morbidity and polypharmacy. Mr Mercer-Jones will outline the initial management options in primary care and then discuss which patients may require further intervention together with what is currently available at QEH.
4 Inflammatory Joint Disease - Early Diagnosis and Associated Co-morbidity
Dr Martin Rynne - Consultant Rheumatologist, QEH
Early diagnosis and treatment of Inflammatory Joint Disease has been shown to decrease long term disability. New treatments are more readily available. Inflammatory Joint Disease has also been associated with vascular disease. Dr Rynne will discuss the early diagnosis of Inflammatory Joint Disease, initial management, use of sub-cut methotrexate and what other co-morbidity clinicians should be screening for in this group of patients.
5 Childrens Continence Services
Michelle Newton - Children’s Continence Advisor SoTW
Urinary and bowel incontinence places considerable strain upon not only the children affected but also on the wider family. School work and relationships are often adversely affected. A new unified service has been developed within the NHS South of Tyne & Wear. Michelle will give an update of the service and what options are available to help these children and their families.
6 Healthcare Acquired Infections [HCAI’s]

HCAI’s are relevant to Gateshead since we have 2.5 times the national average for community C Difficile infection. Also from 2012 all general practices will need to register with the Care Quality Commission [CQC] and as part of that process demonstrate safe infection control practice.
This workshop will look at all the local efforts to reduce both C Difficile and MRSA infection across Gateshead through improved prescribing and clinical care, and help prepare us for the infection control aspect of CQC registration.

Trainer:
Jill Eastham brings to the training her own personal experience of using self-harm to cope with the trauma of reliving painful memories of childhood sexual abuse. She offers part of her own ‘story’ to help you understand the individual meaning and purpose of her own past self-harm and describes the factors that were helpful to her in beginning to reduce and eventually stop that need to self-harm. As a professional counsellor, Jill developed Self-Injury Support in North Cumbria (SIS) into a unique independent service that was valued for its expertise and person-centred ethos. Jill offers professional understanding and insights from her work with the charity to further your knowledge around self-harm. In particular NICE guidelines, case studies and examples of care management are included.

Recent studies show that around 20% of daytime primary care contacts with patients are on the telephone. This figure is rising, affecting us all. Hospitals are using the telephone more to reach out to patients and heavy and increasing use is made of the telephone for emergency care.
The challenge is to understand what the patient’s concerns might be, as well as collecting the clinically important information, without the usual clues available by being able to see the patient.
Unsurprisingly, telephone calls are also a significant source of complaints and even lawsuits. Our seminar introduces some interesting evidence and allows the group to process it with facilitated discussion.
Group exercises allow the participants to examine some of the important messages and to reflect on their own experience, before moving to rehearsal of specific skill areas that will most repay development– you will leave with enhanced skills for handling these contacts with patients.
Communicating About Risks With Patients

Clinical care is risky. Untreated illness carries risk, treatments carry risk and screening procedures bring risks. For a patient to know if a treatment, procedure or screening test is worthwhile, they need to understand these risks, as well as having a clear idea of the chances of benefiting from the intervention in the first place.
The precise communication of risk, in terms that patients can understand, is not an easy task. We balance brief presentation, focused discussion, group work and skills rehearsal – supported by a referenced workbook and moving briskly to retain learner engagement. Participants will leave with their skills enhanced and with a new tool to use in these conversations with their patients.

Children whose welfare is at risk are more likely than their peers to present to doctors and nurses. And doctors and nurses may be the first people to recognise the warning signs of a child at risk of harm or neglect.
With gross abuse, recognition and the right course of action is not hard to discern (although not necessarily easy to execute).
More commonly, we develop a concern, such that further investigation of a child is thought to be helpful. These “greyer” areas can be difficult to share with parents – most clinicians find them uncomfortable and few have received the sort of detailed training that is likely to be professionally useful.
In our seminar, participants’ compare their own experiences. We process some ideas and facts with discussion and group exercises. In common with all EPI training, we then move to practice focused areas of conversations, using suggestions based on best current evidence.
.The health agenda is moving more and more towards prevention of ill health and maintenance of health. So, whether it is the management of a chronic condition (e.g. diabetes, hypertension) or the targeting of specific behaviours (e.g. smoking, drinking, alcohol, and loosing weight) consultations are increasingly likely to address patient behaviour.
However these sorts of conversations are frequently mentioned (by doctors, nurses & allied professionals) as the most frustrating they have with their patients. How can we target our time efficiently whilst retaining effectiveness with patients?
There is an increasing research base to show us which behaviours help.
In this seminar, we introduce some of the most significant evidence and allow the group to process it with facilitated discussion. In particular, the techniques of Motivational Interviewing have much to offer & are explored.

Can we trust the GPs?
Does the public trust the GPs and are they right to do so? Are older people discriminated against in our healthcare system and are GPs party to that? Do GPs take rationing decisions and should they? Are GPs really interested in keeping the population healthy?
GPs have been – along with other doctors – shown to discriminate against some groups of patients. We need to tease out why, and GPs need to be clear with the public. Yet in almost all areas GPs are more trusted than almost all other professionals – and there is a reason for that. This course is directed at GPs and GP SpRs with no or little experience of skin surgery, or those who require further training in basic surgery. The course objectives are based upon the current NICE guidelines for skin surgery in primary care and aim to ensure that the right surgery is done in the right setting by the right person. Locum GPs have always played an important role in general practice. Under the nGMS contract, GPs are required to raise the standard of patient data quality and are looking to locums to support them in this task.
Training provides the essential skills a locum GP needs to complete an electronic patient record.
Working with locum GP agencies and principal GPs, EMIS has devised a training session which will ensure that delegates have foundation knowledge of using the EMIS clinical system during consultations. We are delivering the course in 2 ways; as a full day hands on course at our training centre in Leeds, or as a series of web based training sessions delivered by one of our accredited trainers.

SELF DIRECTED LEARNING GROUPS (SDLGs), BALINT GROUPS (BG) AND CO-MENTORING GROUPS (CMG)
This evening aims to:
-increase awareness of the value of these various kinds of professional groups
-facilitate networking between doctors wishing to set up new groups or join groups
-facilitate sharing of good practice between existing groups
-facilitate collective learning : what have we learn over the the last year about models which work well for self -directed learning groups
What are they and why might I want to join one ?
1) Education and Support. They are groups of practitioners who meet regularly to address their shared professional development needs. Broadly this means meeting for educational purposes and also for peer support. For example:
a) the group may agree a set of priority topics which all are interested in receiving an update on. These are shared out amongst individual group members who bring the prepared topics to help others get up to speed.
b) each member to volunteer to keep track of a certain publication for a few months and report back with important articles.
2) Benefiting from the work of others not just your own. Everyone benefits from the collective efforts of the group rather than each practitioner having to be totally self reliant to keep up to date and feeling overwhelmed by the volume of new publication guidelines and directives published.
3) Tricky subjects are better dealt with by talking: There are particular topics which are difficult to get to grips with without discussion and exploration e.g. policy changes in the NHS such as practice based commissioning, ethical problems, professional development decisions, significant events, and complaints. We all look up different Puns and Dens and learn about new resources.
4) Motivation Because groups set topics and share them out tasks may feel more achievable which helps motivation. Peer expectations also have a beneficial effect on personal habits.
5) Isolation. Practitioners may feel isolated at work either because there are infrequent opportunities for meeting with others during working time, because they are a locum, or they are not at work when practice meetings occur, or they are the only salaried doctor in their practice. Maslow said that according to a “hierarchy of needs” individual performance is improved when people have a sense of “belonging” to a social network.
6) Revalidation is likely to impact differently on different doctors. Locums may be under special scrutiny unless they are seen to work in a managed environment. Forming a SDLG may be an important first step in this process.

'Roleplaynorth Communication Skills'
A simulated patient (sometimes referred to as a “standardised” patient) is someone who has been carefully trained to portray the emotional, symptomatic and physical
characteristics of an actual patient. The simulated patient is able to express the same symptoms and even to demonstrate the same physical examination findings as the “real” patient and in addition is trained to provide feedback to students on their performance during the encounter. These simulations, and the use of role-play, are playing an increasingly important role in the education and assessment of healthcare professionals allowing instructors and examiners to carefully control the clinical learning experience. This session will look at the uses of simulated patients and allow participants to practice setting up and running role-plays.

2 day intensive communication skills course. A small group of up to 8 participants - aimed at doctors from registrar level onwards. Also experienced nurses and nurse practitioners - in prmary or secondary care.
Sessional and locum doctors face particular communication challenges stemming particularly from not having the same opportunities to develop long term relationships with patients. Whilst not always a bad thing, the fact of being new to lots of your patients poses increased clinical and medicolegal risk and can be a challenge to time-effectiveness.
We present the core elements of the Calgary Cambridge framework. We then generate a group agenda and work on the particular needs of the participants.
By the end of two days
- everyone will have had an individual review session
- a wide group agenda will have been addressed
- participants will have working mastery of the Calgary Cambridge framework
We will support you in preparation and we will leave you with a clear direction for continued purposeful development of your effectiveness in communications with patients.

The Royal Victoria Infirmary Skin Club for GP postgraduate education in Dermatology will continue in 2006/2007. We will be holding three meetings on Tuesday 21st November 2006, Tuesday 6th February 2007 and Tuesday 8th May 2007.
At each meeting patients will be demonstrated to groups of GPs and the clinical signs, diagnosis and management briefly discussed. This will be followed by supper and a lecture of interest to general practitioners. GP trainers wishing to develop basic appraiser skills to enable them to carry out more effective appraisal of their GPRs, innovative trainees etc.
GPs wishing to gain skills to carry out appraisals of GP colleagues for their PCT. This first day will provide a basis to enable participants to carry out 2-3 supervised appraisals in the following few months. A second day is planned for Friday 13th October, when more advanced skills will be covered, and experiences from the appraisals carried out can be shared.
The course will be run by Dr Di Jelley, Deanery appraisal advisor, and Dr Fiona Galloway, Cumbria appraiser and GP Tutor, supported by Dr Malcolm Thomas.

The National Sexual Health Strategy requires the provision of basic sexual health care in Primary Care. In response BASHH (British Association for Sexual Health and HIV) has developed a 2-day sexually transmitted infection foundation course (STIF).
This provides multidisciplinary training in the attitudes, skills, and knowledge required for the prevention and management of STIs. The course uses a variety of educational techniques. Topics include national and local epidemiology of STIs and related pathologies; principles of STI service provision; sexual history taking, HIV testing and counselling; Partner notification and management. Each participant also rotates through eight teaching stations on common STI presentations.

The RCGP suggests that, for revalidation, GPs should be able to demonstrate training or reflection in communication skills within the last 5 years – e.g. through videos of consultations or a Calgary Cambridge course. These courses offer you a skills update -to improve your clinical practice and to prepare you for revalidation.
They are run by Effective Professional Interactions Ltd, designed with the specific needs of GPs and revalidation in mind. The sessions are facilitated by Dr Malcolm Thomas, who has a background in General Practice and GP education.

Event Description:
The theme of the afternoon is Every Child Matters. Every Child Matters is a national strategy for improving the health and welfare of all children and young people. It describes five aims, or outcomes, for children. They are that they;
• Be healthy
• Stay safe
• Enjoy and achieve
• Make a positive contribution
• Achieve economic well-being
The workshops are based on these outcomes. They will aim to update knowledge and skills in areas that we may already have some expertise, but also raise awareness (and hopefully interest!) of the role that all members of the primary healthcare team can have in other areas of childrens’ lives that we may have traditionally seen as ‘someone else’s’ responsibility.
A key theme of Every Child Matters is co-operation between all agencies responsible for children and young people, since it is only by improving communication and understanding that the interests of children can best be served. A ‘sub-plot’ to the afternoon will therefore be how to achieve better partnership working.
For a more in-depth look at the background and relevance of Every Child Matters, a double workshop under the heading ‘Enjoy and Achieve’ has been arranged - the synopsis for this two-part workshop below explains a little more about its contents.

This workshop aims to introduce a way of thinking about emotional distresswhich can help us to understand common mental health problems, adjustments to major life events, and the psychological aspects of physical health problems. The content is based upon Cognitive Behavious Therapy (CBT), a psychological therapy which has been shown to be a very effective way of helping people understand and change a whole range of commonly occurring difficulties. By the end of the day, participants should: feel more able to make sense of a range of presenting problems relevant to their work; be more confident in their ability to elicit relevant information; be able to assess a difficulty using a general CBT framework; have developed simple techniques to increase motivation and initiate change. There will be an opportunity to sign up and attend regular profession-specific consultation sessions which will help consolodate the information covered and provide an opportunity to discuss cases and develop skills within a supportive environment. This will be Wallsend Locality’s 12th TimeOut event. This forthcoming event will be looking at a variety of subjects around mandatory training, although there were many other issues we could have covered, we are limited to how many we can run on the day. We feel confident we have selected a variety to suit all your requirements. There will be a variety of speakers coming along to run sessions which have been developed to cater for all members of the primary health care team. Just a reminder regarding the venue we are using (Atmel). This has a strict NO SMOKING and NO ALCOHOL policy, this is not only in the building but for the whole of the site, anywhere within the perimeter fence. This is a very important issue, as because of the nature of the business carried out on the site it is classed as a ‘clean site’. Event Description:
The RCGP suggests that for revalidation GPs should be able to demonstrate training or reflection in communication skills within the last 5 years for example through videos of consultations or a Calgary Communication course. This half day course offers you the opportunity to update your skills both to improve your clinical practice and to prepare you for revalidation. It is run by Effective Professional Interactions Ltd, designed with the specific needs of GPs and revalidation in mind. The sessions are facilitated by Dr Malcolm Thomas, who has a background in General Practice and GP education.
