5. Completing NHS Scheme GP locum forms and obtaining supplies
If you qualify for GP locum medical practitioner membership of the Scheme and wish to pension your income, please;
obtain GP locum forms A and B
read the notes for form(s) GP Locum A and complete it carefully.
send the form(s) GP locum A to the GP or GP Practice(s) you do NHS work for, for certification with your invoice at the end of each month. You will need one form for each GP or GP Practice.
when your certified form(s) GP Locum A is returned, read the notes for form GP Locum B and complete it carefully.
repeat steps 2-4 for each month's invoice(s) for payment.
at the end of each calendar month, close down that month's GP locum form B and send it with your cheque for NHS Scheme contributions and a completed form A for each payment declared on form B, to your host HA / PCT. Your forms and cheque should reach your host HA / PCT no later than the 7th day of the month following the end of the month covered by the form B.
This guidance applies to GP's who may wish to pension their locums work as well as 'Freelance' GP Locums.
Please note:
If you are a qualified GP 'pensioning' GP Locum work for the first time, you will be able to backdate scheme membership from 1 April 2002 to 31 March 2003 by completing the relevant forms. Thereafter you can only apply for NHS GP locum medical practitioner membership of the Scheme up to 10 weeks after the end of a period of locum work. Please make sure that your GP locum forms A and B do NOT list periods of locum work that will have ended more than 10 weeks before your forms and contributions will reach your host HA / PCT. The '10 week rule' also applies to 'Freelance' GP Locums
